Metatarsalgia
A metatarsal pad placed from your impression and a cushioned forefoot — designed to lift and spread the load off the ball of the foot.

By condition · custom orthotics
A precisely placed metatarsal dome to spread the third and fourth metatarsals — for the burning, tingling pain between the toes.
Designed for
Morton’s neuroma is a thickened nerve between the metatarsal heads, most often between the third and fourth. Every step squeezes it, tight shoes squeeze it harder, and the result is a burning or electric pain into the toes and the feeling of a fold in your sock.
Relief means making room. This design builds a metatarsal dome into the shell exactly behind the affected heads — placed from your impression — so the bones are lifted apart and the nerve is no longer pinched with each step. The forefoot is cushioned and the arch supported so less weight tips forward onto it.
Medium shell, precisely placed metatarsal dome, cushioned forefoot, low profile; full length; trims along the guide.
From your kitchen table to our bench
Everything happens at home except the making. Milled and finished by hand in our New York workshop, from domestic and imported materials.
Where it hurts, what you do all day, which shoes. We match one of 25 designs to you — no diagnosis, just a fit plan.
A foam impression kit ships within 1 business day of your order. Press each foot into the foam, close the box and send it back. Five minutes, no appointment; shipping is free both ways.
Your impression is checked the day it lands — that is the day your delivery clock starts. A technician contours arch, heel and forefoot to your foot; the shell is milled to that contour, finished and checked against your impression.
Under 10 business days from the day we receive your impression, tracked the whole way. Counted in business days from the day your impression arrives at our workshop. Slide them in, lace up, walk. 180 days to decide.

The nerve is being squeezed between the heads of two metatarsals. A dome placed just behind those heads lifts and separates the bones so the nerve has room. Position matters to the millimetre, which is why it is set from your impression.
Neuroma for a sharp, burning or electric pain between the third and fourth toes, tingling into the toes, or a click when the forefoot is squeezed. Metatarsalgia for a broader ache under the ball of the foot.
Narrow, tight shoes are part of the problem. Wear a wide toe box; for slim dress shoes, the three-quarter design can carry the dome — note the neuroma in your order comments.
Your orthotics are shaped from an impression of your own feet, not a size chart, and the shell is set to your weight band. We confirm the length from the impression and trim the pair to the shoe type you told us about. If anything is off, a remake is free inside the 180-day guarantee.
A shelf insole is one of three or four shapes chosen to fit millions of feet; it cushions but it cannot hold a heel or contour an arch that isn't its own. A custom orthotic is shaped from an impression of your foot, so the heel cup, arch profile and forefoot relief are yours. If a shelf insole has already solved your problem, keep it — we say so in our guides.
Full-length designs fit sneakers, walking and running shoes, work boots and most lace-ups; remove the shoe's own insole and slide ours in, trimming along the guide if needed. The three-quarter design is for dress shoes and flats where there is no room for a full insole. Each pair is shaped for one shoe type, which is why a second pair for a different shoe is $139.
